Doctor dresses like a hobo - is it unprofessional?

He is a swell guy, works at a free clinic but he wears old worn out brown sweaters with holes in them - not one but three sweaters. In all the years on a bi-weekly basis I've never seen another outfit. He is also a coroner but not on call all the time, so there is no excuse. Should I say something to him?
