Do false-flag terror (or otherwise) operations actually occur?

I've heard about Operation Northwoods and a somewhat shaky claim that the USS Liberty incident was a false-flag gone wrong but are there any real, doented cases? What country perpetrated it? With what political motive? Thanks.
